Users should be confirmed if email confirmations are disabled

Here's what can happen, which I think is wrong:

• Admin starts out with email confirmation turned off
• Many users join the server
• The admin enables email confirmations
• Existing users can no longer log in

I think when email confirmation is turned off, the user should be confirmed automatically when they register. This will also help prevent confusion when looking at the user list in AdminFE.
