double follow from users already who were following

me and at least one other user on miniwa.moe (fence@miniwa.moe) have gotten double follow notifications from external users, so far I've seen this from 2 users, nemo who has definitely been following me for a while, and FloatingGhost@ihatebeinga.live which is from a new instance.
