[UI/UX] Reports and notes

The UI to add notes on a reports is not clear enough.

Here is a list of suggestions:

• Rename the Send button to Add note
• Add a green notification saying Note added to report when clicking this button
• Make the note field required to don't add empty notes

I also suggest this changes for reports:

• Rename Actor to Reporter and move it just below Account
