Internal fetch user creation issue
Each time pleroma starts this happens:

** (Ecto.ConstraintError) constraint error when attempting to insert struct:
* users_nickname_index (unique_constraint)
From what I understand, internal.fetch
user is being created on each start, which causes unique constraint violation.
Probably caused by this !1440 (diffs)
